Notes¶
- Symptom: what the team observed. "Intake stopped indexing after 4 cycles."
- Root Cause: why it happened. "Belt tension too low, jumped teeth under load."
- Fix: what we did about it. "Retensioned; ordered stronger belt."
- Prevention: what stops recurrence. "Preflight torque check every practice session; spare belt in pit."
- Status: Open (fix underway), Fixed (verified in test).
Recurring items (same subsystem, same root cause, ≥ 2 rows) are flagged for design change next season and recorded in the retro.
